At Atchison High School we are going online for all physical forms. Every year regardless of the best intentions we have numerous athletes that have to sit out the first practice because they don't have all their forms filled out correctly and turned into the office. These forms are required forms by the district and KSHSAA. Those forms included KSHSAA PPE Form (the one the doctor signs), KHSAA Concussion Form, Code of Conduct Form, AHS Handbook Form, Emergency Release Form, and Impact Release Form. It is very easy to forget to sign one of these numerous forms in the correct spot.
To help keep this from happening and get all our athletes on the field/court at the beginning of the season we have teamed with our rSchool website and started online registration for athletics and activities. Now parents will go online to fill out and electronically sign these forms. This will allow parents to complete these forms from anywhere (they claim they can also be done on your mobile device, but I have not tried that at this point and time). Parents will still be responsible to fill out the PPE form on paper because that is the form they take to the doctor's office and he/she has to fill out. Parents can even upload a screenshot of this PPE form. Not a 'must do' as you can still bring them to the office, but if you are tech savy enough and would like to upload it there are options for you.
Tips to help you when filling out the online athletic registration:
-Fill in all the fields. If you leave one blank that was required when you hit submit it will pop you back to the page and give you a reminder that you forgot to fill it in, but then will erase all the fields you filled in. This happened to me a couple times.
-Make sure and check mark all the boxes that are required. You will have the same issues as stated above if you forget a box.
-When you create a profile it will ask you to validate your e-mail address. It sends you an e-mail then you have to click on the link to make sure you are not a robot.
